Setting & Preparing for Your Appointment

Please follow these steps to ensure a smooth and on time appointment.

Step 1

Book an Appointment

Schedule Your Appointment

You will receive a confirmation email once your appointment is confirmed.

Step 2

Tell Us Your Health History

Complete your intake (health history) questionnaire online.


After you have received your appointment confirmation via email, you will receive an email with a temporary passcode to fill out your health history information. Please take a moment to fill out the forms and click SIGN & SUBMIT.​​

Step 3

Upload Documents

Upload documents.

Please upload (below, on this page) in the "Add File" box or upload it on our patient portal

  • Uploads are SSL/TLS encrypted for your protection. You can also send us your records via secured fax or text at 240-206-7004.

    • Upload Documents:

      • Photo ID (front and back)

      • Medical records, relevant to the medical condition(s) (no more than four (4) pages)

      • Medication list, if available​

      • ​​If you are trying to receive government assistance, military, first responders, or student discount, you must show proof of identification to receive the discount.

Cancellation Policy:

To avoid a $50 fee, please notify us at least 24 hours prior to your appointment to cancel or reschedule.
