Please follow these steps to ensure a smooth and on time appointment.

Step 1
Book an Appointment

You will receive a confirmation email once your appointment is confirmed.

Step 2
Tell Us Your Health History
Complete your intake (health history) questionnaire online.
After you have received your appointment confirmation via email, you will receive an email with a temporary passcode to fill out your health history information. Please take a moment to fill out the forms and click SIGN & SUBMIT.​​
Step 3
Upload Documents

Please upload (below, on this page) in the "Add File" box or upload it on our patient portal.

Uploads are SSL/TLS encrypted for your protection. You can also send us your records via secured fax or text at 240-206-7004.
